You may have heard it said "accidents happen!" indeed they do; however, in most cases they don't just happen, they are actually, in fact, constructed as part of a sequence. Three basic elements usually exist in the construction of an accident.
 An accident is:
"An unsafe condition or situation, that is compounded or aggravated by an unsafe act or event, which terminates, usually suddenly, with undesirable consequences, e.g. "injury, damage, embarrassment, etc."

The wheel the squeak and the grease

1. If a wheel squeaks then the equipment was not maintained or operated properly.

2. At the squeak of one wheel, check them all.

3. The wheel that doesn't squeak is the one that falls off without warning!
