You may have heard it said "accidents happen!" indeed they do; however, in most cases they don't just happen, they are actually, in fact, constructed as part of a sequence. Three basic elements usually exist in the construction of an accident.
"An unsafe condition or situation, that is compounded or aggravated by an unsafe act or event, which terminates, usually suddenly, with undesirable consequences, e.g. "injury, damage, embarrassment, etc."unsafe condition! unsafe act! undesirable consequences!
